Purchase $25 worth of ConAgra products between 10/12/10 and 1/2/11 and mail in this rebate form with receipts to receive a $30 coupon booklet and a $5 Walmart gift card via the mail. Looks like the products have to be purchased at a Walmart store in order to be valid since they mention "Supercenter" a couple of times in the terms and conditions.
